SUPPLEMENTAL/BID BULLETIN

 

Addendum No. I

 

          This Bid Bulletin is issued to clarify some items and to inform prospective Bidders of the following changes and/or modifications in the Terms and Conditions laid down in the pertinent bid documents for the requirement “Various Medicines” for the use in the Pharmacy Section of the Provincial Health Office, Negros Oriental (T-141-2011, ABC P1,100,000.00)”.  This shall form an integral part of the Bid Documents.

 

  1. 1.     For dextrose – IV hangers should be strong enough to support the weight of the fluids.

 

  1. For dextrose – The protective cap or seal must be easily removed without affecting the sterility of the dextrose.

 

  1. 3.     For Item Nos. 12-16 – All needles should be sharp.

 

  1. 4.     For all Items – Should have the evaluation result

 

In addition, the SUBMISSION AND OPENING OF BIDS which is originally scheduled on OCTOBER 31, 2011 is MOVED on NOVEMBER 2, 2011 (Wednesday) at 10:00 o’clock in the morning.
