SUPPLEMENTAL/BID BULLETIN

 

Addendum No. 1

 

          This Bid Bulletin is issued to inform prospective Bidders of the additional requirements in the technical specifications for the project requirements “Various Medicines” for  use in the Pharmacy Section, PHO, province of Negros Oriental under Bid No. T-73-2012, ABC P2,951,750.00, viz:

 

  • Interested bidder should attach in their Section VII – Technical Specifications a Certification from the TWG that they have submitted samples for Item Nos. 39, 40, 41, 42, 43, 44, 45 & 46.
  • Item Nos. 32 & 56  should be in GLASS AMPULES.
  • Item No. 22 should be in PLASTIC VIALS.
  • Item No. 58 should be RUBBERIZED SEAL.
  • Item No. 36 should be PRESERVATIVE FREE.

 

  • If Sole Proprietorship, the owner of the Company should be the one to execute the Omnibus Sworn Statement and not the authorized representative.
