SUPPLEMENTAL/BID BULLETIN

 

Addendum No. 1

 

          This Bid Bulletin is issued to inform prospective Bidders of the changes in the  schedule for the submission and opening of bids and additional requirements in the technical specifications for the project requirements “Various Medicines” for  use in the Pharmacy Section, PHO, province of Negros Oriental under Bid No. T-52-2012, ABC P3,143,640.00, viz:

 

  • SUBMISSION AND OPENING OF BIDS IS MOVED ON AUGUST 29, 2012 AT 11:00 O’CLOCK IN THE MORNING AT THE SAME VENUE.

 

  • Interested bidder should attach in their Section VII – Technical Specifications a Certification from the TWG that they have submitted samples for IV CATHETERS.

 

  • If Sole Proprietorship, the owner of the Company should be the one to execute the Omnibus Sworn Statement and not the authorized representative.
