This Bid Bulletin is issued to inform prospective Bidders of some changes and/or modifications introduced by the Bids and Awards Committee for the requirements “Various Medicines for the use at Pharmacy Section for various patients of NOPH under Bid No. B-43-2015, ABC:P15,813,450.00, viz:
Additional Requirements:
- In addition of the requirement that the bidder must submit Certificate of Bio-equivalence/Bio-availability/PO from UST Hospital, PGH, Makati Med., and Saint Luke Hospital for its acceptability (for items nos. 14,16,18,20,22,26,28,58,60,64,74,89,104,106,107 & 109), the bidder should also attach another CERTIFICATE of Bio-equivalence issued by Food and Drug Administration (FDA).
- The bidder should issue a Letter of Guarantee if the medicines they are going to deliver will expire in less than two (2) years. However, medicines delivered with expiration of less than one (1) year is not acceptable by the end-user.
Bidders are enjoined to take note and consider the following changes in the preparation of their bids.
