The Province of Negros Oriental is planning to procure the following requirements “ One Lot – 2,750 bags Organic Fertilizer & 300 liters Pro-biotic Soil Conditioner” for use for System of Rice Intensification (SRI) Bio-Organic Farming Project of Task Force Guard, province of Negros Oriental thru the mode of procurement “Negotiated Procurement” as a result of Two Failed Biddings, in accordance with Section 53.1 of the IRR of R.A. 9184:
In this connection, you are invited to submit your quotation for the proposed project which includes the following:
Eligibility Documents – Class “A” Documents
- SEC Registration or DTI Registration
- Mayor’s permit
- Statement of all its ongoing and completed govt. and private contracts, including awarded but not yet started
- Audited Financial Statements stamped “received” by the BIR for FY 2011
- NFCC computation or Credit Line Certificate
Technical Documents –
- Bid Security – 2% of the ABC if in the form of Cash or Cashier’s/manager’s check or bank draft/guarantee or irrevocable letter of credit or 5% of the ABC if in the form of Surety Bond issued by insurance company duly certified by the Insurance Commission as authorized to issued such security
- Conformity with technical specifications and schedule of requirements (Using the attached form)
- Omnibus Sworn statement
Financial Component –
- Financial Bid Form, which includes the price schedules (Using the attached form)
You are required to submit the abovementioned requirements on July 5, 2012 (Thursday) at 10:00 in the morning at the Old SP Session Hall,CapitolDumagueteCityduring the negotiation conference.
